Registration Page
Use this page to register for any of our group dog training classes. Simply select the class that you would like to take from the choices offered below and then use the confirmation page to pay for your selection. Please note, your space in class is not reserved until payment and registration form are complete!
If you have questions, would like to observe a class, or need help selecting your class, don't hesitate to call us at 512-636-3793.
******Important: All dogs entering a group class must provide proof of Parvo, Distemper, and Rabies vaccinations. Please have your veterinarian call us with this information at 512-636-3793 or fax your dog's vaccination records to us at 512-323-9480 before the start of your class. If your dog is under the age of 16 weeks, your veterinarian must sign this instead - they should fax this form to the same number. Thank you!
General Agreement: In consideration of the acceptance of this registration, and the holding of classes, and the opportunity to have the dog participate, I agree to hold Buddy’s Chance, LLC, Carolyn Shannon, and all trainers, guest trainers, independent contractors, employees, assistants, or apprentices, the premises upon which the classes are to be held, harmless from any claim for the loss or injury, both direct and indirect, to any person or thing by any act of dog or person while in or upon the premises or grounds or near any entrance thereto, unless the injury was caused by the negligence of the above named persons. I personally assume all responsibility and liability for any such claim. I further agree to hold the aforementioned parties harmless from any claim of loss of this dog unless such loss was caused by the negligence of one of the above named parties. I hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expenses, including legal fees, by reason of the liability imposed by law upon any of the aforementioned parties for damage and expenses. PAYMENT IN FULL IS REQUIRED TO HOLD YOUR SPACE IN CLASS. THIS IS A NON-REFUNDABLE, NON-CANCELABLE REGISTRATION - REFUNDS OR MAKEUP SESSIONS WILL BE CONSIDERED ON A CASE-BY-CASE BASIS AT OUR DISCRETION. PLEASE CONTACT CARA at 636-3793 TO DISCUSS ANY ISSUES REGARDING MAKEUP CLASSES OR CHANGES IN CLASSES.
Photo/E-mail Release: Signature allows the aforementioned parties to use videos and photos of you and/or your dog for educational and promotional purposes unless stated otherwise and to contact student via e-mail.
