Thank you for registering for services with Buddy's Chance! To complete your registration, please:
1) Ask your veterinarian to fax proof of vaccinations to us at 512-323-9480 or, in the case of a puppy under 4 months, ask them to sign and fax this form to us: vaccination form.
2) Select your class, daycare, boarding, or grooming and complete the form below. Please note that clients who have already completed our form for a previous class or for dog daycare and boarding may use the express registration function. If you need to register more than one dog, simply fill out the form for one dog and the system will bring you back to the registration section at the end. Then, select the class or service you need for the second dog and use the Current Client Quick Sign Up function and choose ADD NEW DOG for the second dog. Thank you!
