Use this page to register for our dog daycare or boarding service after you book your daycare or boarding with us. If you still need to nook daycare or boarding or ask a question, you can call us at 512-343-8100 or email us.

After completing this page, please ask your veterinarian to fax your dog's proof of vaccination to 512-323-9480. Thank you!

General Agreement: In consideration of the acceptance of this registration, and the daycare or boarding services provided, and the opportunity to have the dog participate, I agree to hold Buddy’s Chance, LLC, Carolyn Shannon, and all trainers, guest trainers, independent contractors, employees, assistants, or apprentices, the premises upon which the classes are to be held, harmless from any claim for the loss or injury, both direct and indirect, to any person or thing by any act of dog or person while in or upon the premises or grounds or near any entrance thereto, unless the injury was caused by the negligence of the above named persons. I personally assume all responsibility and liability for any such claim. I further agree to hold the aforementioned parties harmless from any claim of loss of this dog unless such loss was caused by the negligence of one of the above named parties. I hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all loss and expenses, including legal fees, by reason of the liability imposed by law upon any of the aforementioned parties for damage and expenses. 

Photo/E-mail Release: Signature allows the aforementioned parties to use videos and photos of you and/or your dog for educational and promotional purposes unless stated otherwise and to contact student via e-mail.

Veterinary Release: I agree that Buddy's Chance may take my dog to my veterinarian or a veterinarian of their choice if they have reason to believe that my dog is injured or ill. I agree to reimburse Buddy's Chance for any veterinarian bills incurred up to $500 and for their time in taking my dog to the veterinarian (at a rate of $25 per hour). I understand that Buddy's Chance will contact me immediately in the event that my dog needs medical care. I do not authorize any veterinarian to euthanize my dog without my express written permission and do not authorize Buddy's Chance to give such authority to any veterinarian.
